Eric McNair Scott, Principal of Community Clinical Advances,  works at the intersection of community health, health care, and social services.  With leadership experience directing Waiver 1115 systems change initiatives and a background in strategy development, management of care coordination programs, and direct care, Eric provides capacity and support to community organizations, clinical organizations, and Community Care Hubs. He supports partners in planning and implementing systems changes to increase community impact, advance equity, and support sustainability.

Eric  participates with national cross-sector leaders in Partnership to Align Social Care (P2ASC), a learning and action network to co-design strategies for successful partnerships to align social care and health care. He led Waiver 1115 initiatives and systems change efforts between 2017 and 2023 as Director of Community and Clinical Linkages with the Southwest Washington Accountable Community of Health (SWACH).

Under Eric’s leadership at SWACH, SW Washington implemented the infrastructure and network of partnerships for a successful Community Care Hub, the HealthConnect Hub, supporting over 6000 community members to connect to needed health and social supports in Clark County, Skamania County, and Klickitat County
